The Benefits at a Glance document is an abbreviated summary document. For the interpretation of specific benefits, the actual plan document shall be used. Some benefits are pro-rated for part-time Associates. Mercy Health Partners (MHP) provides a full compliment of benefits which, combined with your paycheck, represents a total compensation commitment.

Health Insurance MGHP and Associate 24+ hrs per week budgeted; 1st of month following 30 days of employment Blue Care Network (BCN) HMO or Trinity Health PPO Plan (administered by BCBSM)
Health Insurance Opt-Out MGHP 32+ hrs per week budgeted; 1st of month following 30 days of employment $1,000 per year (40 hours/week)

Dental Insurance MGHP and Associate 24+ hrs per week budgeted; 1st of month following 30 days of employment 100% preventative, 50%-80% of other services - $1,000 annual maximum per covered member. Orthodontics $1,000 lifetime maximum
Vision Care Associate 24+ hrs per week budgeted; 1st of month following 30 days of employment Co-Pays for exam and materials
Life Insurance MGHP 24+ hrs per week budgeted; 1st of month following 30 days of employment One and one-half times annual base compensation

Optional Life Insurance Associate 24+ hrs per week budgeted; 1st of month following 30 days of employment Additional life insurance for Associate, spouse, and dependents
Short Term Disability MGHP 24+ hrs per week budgeted; 1st of month following 90 calendar days 66 2/3% of base salary for duration of short- term disability up to 26 weeks. 7 day elimination period; (Effective 7/1/05)
Long Term Disability MGHP 24+ hrs per week; After Short Term Disability Per insurance policy provisions
Flexible Spending Accounts Associate Upon employment Tax-free dollars for expenses in either Health Care, (some over the counter medications reimbursable), Premium or Dependent Care
Group Legal Plans (Hyatt) Associate 24+ hrs per week budgeted; 1st of month following 30 days of employment Access to professional legal representation
Health Club Membership (Example: YMCA, Omni Fitness, and Norton Pines Athletic Club) Associate 16 + hrs per week budgeted; Upon employment Discounted fees; payroll deducted; (contact health club directly for specific information)

Retirement Program (Transferable to all Trinity Health Membership Organizations) MGHP Upon employment Defined Benefit Plan (non-contributory) for monthly pension upon retirement. See Summary Plan Description. 100% match on the first $500; .50˘ on the dollar thereafter, up to 2% of gross income per year
Retirement Savings Plan Associate (Voluntary); Upon employment Trinity Health - TSA / 403(b) Retirement Savings Plan
Insurance Plan Associate Upon employment Insurance for auto, home and recreation vehicles
Educational Assistance MGHP After 1 year Financial support based upon budgeted hours. Up to $2,000 per fiscal year (July1 –June 30) for full-time; $750 for part-time. Must obtain a “C” or better.
Payroll, Direct Deposit Associate Upon employment Payroll deposit and deduction options for specific amounts at various financial institutions
Professional Liability MGHP Upon employment Per insurance policy provisions
Overtime Pay MGHP Upon employment, with authorization 1-1/2 x hourly rate for all hours worked over 40 in work week
Shift Differential MGHP Upon employment Extra compensation paid for work on designated shifts
Saturday/Sunday Premium MGHP Upon employment Extra compensation paid for hours worked on Saturday/Sunday
Holiday premium MGHP After 90 days Per contract provisions
Mandated Hours MGHP Upon employment Two times average hourly rate for all mandated hours worked
On-Call Pay MGHP Upon employment Extra compensation paid to be available for work as authorized
Call-In Pay MGHP Upon employment 1-1/2 times base rate for minimum of 2 hours
Bereavement MGHP After 90 days Up to a maximum of 24 scheduled hours

Associate Assistance Program MGHP Upon employment Assessment and counseling visits for Associate and household members
Rest Periods MGHP Upon employment 1 – 15 minute rest period per 4 hours worked
Parking MGHP Upon employment Free parking in designated areas adjacent to hospital
Associate Discount Associate Upon employment 20% discount on balance of inpt/outpt. Charges not covered by insurance plan
Associate Service Awards MGHP 5 year intervals Gifts awarded to qualified Associates
Recreational Activities MGHP Upon employment Participation in a variety of recreational activities
Associate Health Services MGHP Upon employment Employment physical examination. Medical treatment for on the job injury or illness
Worker’s Compensation MGHP Upon employment Loss time benefits and coverage of medical expanses in accordance with State Law
Social Security MGHP and Associate Upon employment Income payment at retirement or in case of disability. Employer match of Associate contributions
Unemployment Compensation MGHP Upon employment Income replacement in accordance with State Law
