Here to Help You
Choosing a nursing home is one of lifes most difficult decisions.
You want to be sure youre doing the right thing, and know
that the place you select provides the quality of care you want.
Thats why so many families choose a Mercy Health Partners
Living Center.
The Living Centers are sponsored by Trinity
Continuing Care Services and are designed for the comfort, support
and security of our residents. Our professionals provide services
that meet the highest standards of quality. And our care reflects
concern for the independence and dignity of every person who seeks
our help.
A Long Tradition of Caring for the Total Person
You will find that we are different. The Sisters of Mercy trace
their roots back to their foundress in Ireland, Catherine McAuley.
Catherine was a deeply religious, but very practical woman. She
knew that the total person must be served, and that included all
of their physical and spiritual needs. Mercy Health Partners
carries that tradition forward to the present day.
Planning Together to Meet Your Needs
We like to begin with a partnership that evaluates your needs and
brings you the services you need without generating unnecessary
and unwanted expenses. Well discuss options and solutions
with you, then assist with arrangements.
Access to an Integrated Delivery Network
The network of services we provide is called an Integrated Delivery
Network. Unlike any other provider of services, we will arrange
care that serves your needs, not ours. Since we manage a full array
of health care organizations, we can easily arrange and implement
a care plan that meets your needs in an environment as familiar
as possible.
Encouraging Independence
Being able to live independently brings a sense of self-worth and
dignity. During your stay with us you will notice that we work very
hard to cure your ills and/or give you the tools and training to
return to a more independent living situation.
Close to Home
Weve been a part of your community for years, providing healthcare
services for every age and stage of life, including later years.
You can rely on Mercy Health Partners for high quality care,
from physicians and nurses you know.
If you or a loved one will be coming directly to the Living Center
after a hospital discharge, well consult with hospital personnel,
including your physician, to make sure that the transition is smooth.
Caring for Individuals
In long-term care, as in all our services, the individual is our
focus. We recognize that each person has unique needs. They are
not only medical needs, but emotional and spiritual as well.
Our care plans place heavy emphasis on rehabilitation. Once you
enter our system, we will set goals for improvement in your abilities
to function independently.
Outstanding Professional Staff
Only persons sincerely interested in working with older adults
are employed at the Living Centers. Our staff members are carefully
selected and continuously trained in their specialties. We draw
on the talents of all caregiversphysicians, nurses, aides,
counselorsto ensure that individual emotional and physical
needs are attended to.
Physician Involvement
Personal relationships are fundamental to our care. Each resident
has his or her own physician who works with our professional staff
to develop and carry out individual care plans. Physicians consult
regularly with family members. Mercy Health Partners has
thousands of physicians, so when specialized knowledge is needed,
its usually available.
Family Involvement and Support
We understand the deep concern that families have about entrusting
their loved one to the care of others. At the Living Centers, weve
established support groups to help family members through difficult
times. Shared experiences with other families offer reassurance
and strength thats not available elsewhere.
