Palliative Care

Palliative Care is a collaborative effort between Mercy Health Partners and Harbor Hospice. Palliative Care is an inpatient consultation that provides services that seek to prevent, relieve, reduce or soothe the symptoms of a life-limiting disease or disorder. The goal of this program is to enhance the quality of life for the patient as well as being a resource for the family.

An interdisciplinary team consisting of a Medical Director, Nurse Coordinator, Pastoral Care and Social Work have been formed. This team of professionals works with both the patient and their family to provide the needed resources during this crucial time. Additionally, the team works hand in hand with physicians and staff to ensure quality care and consistent communication is provided.
