Medical Social Work Services
Social Work in the MHP ER
You and your family are worried about Mom. She is living all alone,
falls frequently, and appears isolated and alone. Sometimes you
wonder if she is taking all those medicines as prescribed. You cannot
provide the 24-hour supervision you feel she needs. Late one night
you get a call and she has fallen, she is brought to the MHP Emergency
Room. Thank goodness she’s not seriously hurt , and her injuries
don’t require a hospital stay, but how are you going to help
make sure she’s safe?
Social workers are available 16 hours a day seven days a week to
assist families and patients with health related problems. We can
help you understand the maze of insurance and how to search for
living situations where your loved one is safe and well cared for.
We can arrange for services in the home, help obtain medical equipment,
and refer you to agencies in the community for on-going support.
Crisis Intervention
We are here to assist with mental health and/or substance abuse
crisis. We can refer for inpatient and outpatient treatment. We
can direct you and your family to local support groups.
Services to the MHP Primary Care Network
We are available to work with your primary care physician and assist
with placement in alternative living environments. We can coordinate
services in your or your family member’s home. If you know
you are going to be in the hospital we can help you prepare for
your discharge, even before you are admitted! Just let your doctor
know you are interested in talking to a MHP Social Worker.
Acute Rehabilitation
A social worker is available on our inpatient Acute Rehabilitation
Unit. The social worker is the primary contact between the your
family and the treatment team. The social worker helps to make the
transition from hospital to home as comfortable as possible. If
it is felt you or your family member is not ready to go home yet,
the social worker is there to assist in finding a place where you
may get further rehabilitation.
Inpatient Social Work
In collaboration with nurse case managers, physicians, physicians
assisstants and other members of your treatment team, the MHP
inpatient social worker will assist in planning for your discharge.
Social work partners with various community agencies/resources
to assist you in your post hospital recovery and maintaining
your health once you recover. Patients and families are free
to request our services any time during the inpatient stay.
